Provide your contact and demographic information

We ask you to provide us with your contact information so that an investigator can reach out to you to discuss what happened. We also ask for some basic demographic information to help us understand on a larger scale who may be most often affected by incidents of harassment and discrimination - to help us with developing prevention measures.

Provide details about the incident you would like to report

We ask you to provide us with any details you can remember about the incident(s) you are reporting, including the date, time, and location.

Provide details on the person about whom you are filing a report

We ask you to provide us with any identifying information that you have about the named party(ies) in your report.

Provide details on anyone who may have witnessed the incident

We ask you to provide us with any identifying information that you have about any witnesses to the incident(s) you are reporting.

Include attachments of any evidence you would like to provide

We will review your report and assign the report to an appropriate investigator to handle. The investigator will speak with you, the named party(ies), and any witnesses identified in your report. The investigator will review the supporting documents you provide and take other necessary steps to gather material and information about the incident(s) you reported. At the conclusion of the investigation, the investigator will make a recommendation on a suitable resolution to your report. You will be notified of the completion of the investigation and provided with a summary of the outcome.
