Reporting Process

Provide your contact and demographic information

By providing us with your contact information, an investigator can reach out to you to discuss what happened. During your conversations with an investigator, we will ask for some basic demographic information to help us understand on a larger scale those who may be most often affected by incidents of discrimination, harassment, and retaliation – to help us with further developing prevention measures.

Provide details about the incident you would like to report

Provide as much information as you can recall about the incident(s) you are reporting, including the date(s), time(s), and location(s). We have incorporated maps to help with identifying locations. Do not worry if you cannot remember everything. You will have an opportunity to speak with an investigator after submitting your report.

Provide details on the person about whom you are filing a report

We ask you to provide us with any identifying information that you have about the named party(ies) in your report.

Provide details on anyone who may have witnessed the incident

We ask you to provide us with any identifying information that you have about any witnesses to the incident(s) you are reporting.

We ask you to provide us with this additional information to aid us in best addressing and assigning your complaint for investigation.

Review and Submit

You will have the opportunity to review the information you have entered and to make changes before you submit your report. Once you have submitted your report, you will receive a report number, have the ability to print your report, and receive information regarding the next steps.

You can use this portal to file a report of discrimination, harassment, and/or retaliation.
